[TiddlyWeb] caching issues

When reloading the document after creating/modifying/deleting a tiddler, TiddlyWeb often serves the previous (cached) version of the entire document.

Steps to reproduce:

  1. load TiddlyWeb-generated TiddlyWiki document
  2. create (or delete) a tiddler, saving to the server
  3. reload the document

The resulting document will in most cases be missing the newly-created tiddler (or retain the deleted tiddler).
A force-refresh will then display the proper latest version of the document.

Random guess: This issue might be related to the time elapsed since the last modification, as both Mike and myself often apply quick changes and then immediately reload.
